Order Form Course Videos

for renting a DVD or Vimeo link
The recordings can only be ordered by the participants of a course

Please fill out the form completely and read and accept the lending conditions:

Duration of lending courses as a Vimeo link 

4 weeks for entire modules
2 weeks for individual course days


Costs
CHF 20 per course day

Payment
cash on collection / within 30 days with invoice (on dispatch)

Conditions DVD rental

  • Return of the DVD's within the agreed time window
  • Obligation not to copy DVD and to respect the copyright of Polarity Education Centre
  • DVD's can be picked up at the office. A shipping fee of CHF 6 is charged for shipping.
  • If the loan period is not observed, the borrower is obliged to pay a fee of CHF 20 per additional week
Conditions Vimeo-Link

The password expires at the end of the rental period. A new rent is again subject to a fee.

Confidentiality/Ethics

As in the courses themselves, the therapeutic ethics rule also applies to viewing study DVDs or Vimeo links: all personal data must be treated as confidential. By placing an order, the borrower undertakes to abide by these rules.
